Thirumangai Alvar sang pasurams for this kshetra in his Periya Thirumozhi as part of his comprehensive circuit of all 108 Divya Desams. The name Thiruvizhimizhalai ('the sacred water with divine eyes' or 'the water-girdled sacred town') evokes the watery, delta landscape of the Nagapattinam coastal belt. The Nagapattinam district's eleven Divya Desams cluster along the fertile Cauvery delta, and Thiruvizhimizhalai is one of the smaller, more rural temples in this group. The Chola dynasty's deep patronage of Vaishnavism in this delta region ensured that even small rural shrines were endowed, maintained, and celebrated in Alvar hymns. The temple continues to serve the local Vaishnava community as a living place of worship.

Thiruvizhimizhalai is in Nagapattinam district, accessible from Nagapattinam town (approx. 30–40 km) or Sirkazhi. Nagapattinam is connected by rail and road from Chennai, Thanjavur, and Trichy.
